Introduction

We respect your privacy and are committed to protecting it through our compliance with this Policy. This Policy describes the types of information we may collect from you or that you may provide when you visit the website atlasmh.com or use the Atlas Parent Green Folder Application (our “App”), and our practices for using, maintaining, protecting and disclosing that information.

This Policy applies to any information that we collect on the App, including any information in any e-mails, posts, and other electronic messages between you and the App and any information about you transferred to the App from other websites. This Policy does not apply to any other information that we collect by any means.

Please read this Policy carefully to understand our policies and practices regarding your information and how we will treat it. If you do not agree with our policies and practices, you can choose not to use our App. By accessing or using our App, you agree to this Policy. This Policy may change from time to time. Your continued use of the App after we make changes is deemed to be acceptance of those changes, so please check the Policy periodically for updates.

What Is Considered “Personal Information”?

Information we collect on our App falls into one of two categories:

  • Information by which you may be personally identified, such as first and last name; home or physical address; e-mail address; telephone number, including a mobile telephone number; payment information such as credit card and billing information; and any other information collected that is directly connected to one or more of the categories of information listed here.

  • Other information that is about you but does not identify you personally.

    For the purposes of this Policy, information listed in the first category above is considered to be your “Personal Information.” All other information that does not identify you personally or that is “de-identified” is NOT considered Personal Information.

Information We Collect About You and How We Collect It

Direct Collection

We may collect several types of information from and about users of our App, including information by which you may be personally identified, such as name, screen name, postal address, e-mail address, telephone number. The information we collect on or through our App may include information that you provide by registering to use our App, using our services, posting material, requesting further services or reporting a problem with our App. We will also retain records and copies of your correspondence (including e-mail addresses), if you contact us.

User Contributions

If you are a provider or program, you also may provide content, materials and/or a photograph of yourself (collectively, “User Contributions”) to be posted, published or displayed on public areas of the App, such as in your online profile. Such User Contributions are posted on the App and transmitted to others at your own risk. We cannot control the actions of other users of the App with whom you may choose to share User Contributions.

Automatic Collection

As you navigate through and interact with our App, we may automatically collect certain traffic data and usage information to help us manage our content and improve your use of the App. This may include information about your equipment, your IP address, your general geographic location, and your browsing actions and patterns when you visit our App, including the referring and exit pages. The technologies that we use for this automatic data collection may include cookies that are stored on your computer, small embedded electronic files known as web beacons (also referred to as clear gifs, pixel tags and single-pixel gifs) and other types of embedded code on the App that permit us to count users and obtain other related App statistics (for example, recording the popularity of certain Website content and verifying system and server integrity).

If you choose not to provide certain information, you may be unable to use our Website and we may be unable to provide certain services that you request.

How We Use Your Information

We use information that we collect about you or that you provide to us, including any Personal Information, to:

  • Present our App and its contents to you;

  • Aid you in connecting with providers or programs;

  • If you are a provider or program, to help users of the App connect with you and share your User Contributions with other users of the Website at your direction;

  • Provide you with information and services that you request from us:

  • Provide you with important e-mail communications, including informational messages or newsletters, information about transactions carried out through the App or services you requested from us, or about updates to this Policy;

  • Analyze how you and other users use our App and respond to the ads displayed on our App, including in conjunction with third party analytics service providers we have hired to assist us with such analysis (as described below in the section titled “Online Tracking”);

  • Permit third parties, including advertisers, ad networks and servers, content providers, and application providers, to provide you with interest-based (behavioral) advertising or other targeted content (as described below in the section titled “Online Tracking”);

  • Fulfill any other purpose for which you provide the information;

  • Fulfill any other purpose that we describe when you provide the information;

  • Notify you about changes to our App or any services we offer or provide though it;

  • Carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including our Terms of Use [www.atlasmh.com/atlasgreenfoldertc]; and

  • For any other purpose with your consent.

    The traffic data, usage details, and “persistent identifiers” (e.g., IP addresses) we collect help us to improve the App and to deliver a better and more personalized experience by enabling us to:

  • Estimate our audience size and usage patterns.

  • Learn to improve the “flow,” organization and user interface of the App.

  • Recognize you when you use the App and otherwise store information about your preferences, allowing us to customize the App according to your individual interests.

  • Speed up your searches.

    We use the feedback you provide in response to surveys you complete to improve the functionality of the App and ensure our expectations for quality service are being met by our employees and other service providers you may interact with through the App.

Use of Anonymized Information for Data Analytics Purposes

We may collect and use anonymized or de-identified information (not including any Personal Information) about users of the App to analyze and track certain trends. We may disclose, sell, lease or license related anonymized data and statistics to third parties without restriction.

Online Tracking and How We Respond to “Do Not Track” Signals

Tracking involves the use of cookies, web beacons or other embedded code or tracking technologies to collect, analyze and store information on a user’s behavior over time on multiple sites, including information on the sites visited, products viewed, products purchased and other online interactions. Tracking information can be used to enable companies to make interest- based (behavioral) advertising available to users on multiple sites that they visit.

We may collect information about your on-line activities over time and across third party sites for tracking purposes when you use our App or use our services. In addition, other parties, including third-party advertisers, ad networks, ad servers, Internet providers and mobile device providers may use cookies, web beacons or other embedded code or tracking technologies to collect information about you when you use our App, and about your online activities over time and across different websites and other online services. They may use this information to provide you with interest-based (behavioral) advertising or other targeted content. We do not control these third parties’ tracking technologies or how they may be used. If you have any questions about an advertisement or other targeted content, you should contact the responsible provider directly. Some of these third parties may offer you a way to opt-out of this type of targeted advertising through industry choice mechanisms. You also may be able to opt out from targeted advertising through the advertising settings for your mobile device.

Analytics providers are third-party companies that collect information when you use our App. These companies use cookies, web beacons, software development kits (SDKs), and similar technologies to help us collect and analyze crash reports, keep track of what content or ads you view, how long you spend on different pages, how you arrived on a particular page and how you respond to the ads we show you. The analytics providers with whom we work may include Google Analytics and Amazon Web Services. Some of these companies, including Google Analytics, give users choices about the use of your information for such purposes, or permit you to opt out of the use of your information for such purposes by taking certain steps such as downloading a browser add-on. To learn more about such choices that may be available to you, we encourage you to contact the responsible service provider directly.

Some web browsers may transmit “DO NOT TRACK” signals. The Website does not currently respond to any “DO NOT TRACK” signals sent by your browser or otherwise.

Disclosure of Your Information

We may disclose your information, including your Personal Information:

  • If you are a provider or a program, to other users of the App to the extent you choose to include it in User Contributions you post or publish publicly, in each case as you direct us to do so via the App’s forms and interactive features designed for this purpose;

  • To third parties (NOT including any Personal Information) including advertisers, ad networks and servers, content providers, and application providers as described above in the section titled “Online Tracking;”

  • To third parties (NOT including any Personal Information) for the purposes described above in the section titled “Use of Anonymized Information for Data Analytics Purposes;”

  • To our directors, officers, employees, subsidiaries, parent and affiliates;

  • To contractors, service providers and other third parties we use to support our business and help us operate, maintain and improve the App, including third party analytics service providers as described above in the section titled “Online Tracking,” in each case who need to know such information to support our permitted uses of Personal Information in accordance with this Policy;

  • To a buyer or other successor in the event of a merger, divestiture, restructuring, reorganization, dissolution or other sale or transfer of some or all of the Company’s assets, whether as a going concern or as part of bankruptcy, liquidation or similar proceeding, in which Personal Information held by us about our App users is among the assets transferred;

  • To comply with any court order, law or legal process, including responding to any government or regulatory request;

  • To enforce our rights arising from any contracts entered into between you and us, including our Terms of Use [www.atlasmh.com/atlasgreenfoldertc];

  • If we believe disclosure is necessary or appropriate to protect the rights, property, or safety of the Company, the App’s users or others;

  • For any other purpose disclosed when the information is provided; and

  • For any other purpose with your consent.

    We may share anonymized or de-identified information—such as traffic data (e.g., audience size and usage patterns) that we collect from cookies, web beacons or other embedded code as well as de-identified or aggregated data and statistics about users—that is not tied to any Personal Information with third parties without restriction.

Social Media Sites and Widgets

If you log onto the App through Facebook or a similar social media site, certain information about you will be transferred to the App, and the fact that you have logged onto the App and some of that transferred information may be available to other users of the social media site. That information may be subject to the privacy policies of any such social media site, and you should review those policies before using a social media site to log onto the App. Any such transferred information will otherwise be subject to the terms of this Policy.

You may be able to use social media widgets such as the Facebook Like button on the App. These widgets will collect your IP address and identify which page you are visiting on the App, and make your preference information available to third parties. The information collected by such widgets is controlled by third party privacy policies.

Choices About How We Use and Disclose Your Information

You have certain choices regarding the Personal Information provided to us:

  • You can set your browser to refuse all or some browser cookies. If you disable or refuse cookies, some parts of the App may then be inaccessible or not function properly.

  • If you no longer wish to receive certain informational e-mails from us, you can opt-out of such communications by clicking on the “Unsubscribe” or “Updating your e-mail preferences” link in the most recent e-mail you received from us and follow the prompts that appear. This opt-out does not apply to any transactional e-mails you may receive from us (e.g., confirmation of registration, responses to inquiries you have addressed to us, updates to this Policy, etc.).

  • In some cases, you can choose not to allow third party analytics service providers to collect and track data about you by opting out and/or installing certain add-ons to your browser, as described above in the section titled “Online Tracking.”

  • In some cases, you can choose not to allow third party advertisers to provide you with targeted advertising, as described above in the section titled “Online Tracking.”

Accessing and Correcting Your Information

You may review, ask us to change, ask us to correct, or request the deletion of your Personal Information you have provided via the App at any time by sending us an e-mail at team@atlasmh.com. If you are a provider or program and you have submitted User Contributions to the App, you may also review, change, correct or delete such information by using the interactive features of the App designed for this purpose. We may not be able to delete certain portions of your Personal Information except by also deleting your user account. We may not accommodate a request to change information if we believe the change would violate any law or legal requirement or cause the information to be incorrect.

If you are a provider or a program and you delete your User Contributions from the App, copies of your User Contributions may remain viewable in cached and archived pages, or might have been copied or stored by other App users.

Your California Privacy Rights

California Civil Code Section 1798.83 permits users of our Website who are California residents to request certain information regarding our disclosure of Personal Information to third parties for their direct marketing purposes. To make such a request, please send an e-mail to team@atlasmh.com.

Users Outside of the United States

Our App is hosted in the United States and our services are provided from the United States. It is possible that certain information will be stored on servers in multiple other countries on the “cloud” or other similar distributed hosting platforms. If you are a resident of any country other than the United States, please note that by providing your Personal Information to us, you are consenting to the transfer of your Personal Information to the United States and other jurisdictions as indicated above, and to our use and disclosure of your Personal Information in accordance with this Policy.

Children

Our Website is not intended for children under 13 years of age, and no one under age 13 may provide any information to this App. We do not knowingly collect Personal Information from children under 13. If you are under 13, do not use or provide any information on this App, use any of the interactive features of this App or provide any information about yourself to us, including your name, address, telephone number, e-mail address or any screen name or user name you may use. If we learn we have collected or received Personal Information from a child under 13 without verification of parental consent, we will delete that information. If you believe we might have any information from or about a child under 13, please contact us at team@atlasmh.com. Our App may be used by children under the age of 18 only with parental consent.

Data Security

We have implemented measures designed to secure your Personal Information from accidental loss and from unauthorized access, use, alteration and disclosure.

The safety and security of your information also depends on you. When we have given you (or when you have chosen) a password for access to certain parts of our App, you are responsible for keeping that password confidential. We ask you not to share your password with anyone. We urge you to be careful about giving out information in public areas of the App such as provider or program profiles. The information you share in public areas may be viewed by any user of the App.

Unfortunately, the transmission of information over the Internet is not completely secure. Although we have implemented security measures that we think are adequate, we cannot guarantee the security of your Personal Information transmitted to our App. Any transmission of Personal Information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the App.

Changes to Our Privacy Policy

We post any changes we make to our Privacy Policy on this page. If we make material changes to how we treat our users’ Personal Information, we will endeavor to notify you by e- mail to the primary e-mail address specified in your account or through a notice on the App home page. The date this Policy was last revised is identified at the top of the page. You are responsible for ensuring we have an up-to-date active and deliverable e-mail address for you, and for periodically visiting our App and this Policy to check for any changes.
